Why do my fingers and lips feel tingly after thyroid surgery?

This is the most commonly alarming symptom after total thyroidectomy and has a specific cause: low calcium. The parathyroid glands (four tiny glands attached to the thyroid) are sometimes temporarily affected by surgery and produce less parathyroid hormone, which controls calcium levels. Tingling in the fingers, toes, and around the mouth is the first symptom of low calcium. This is why calcium supplements are prescribed routinely after total thyroidectomy. If you have tingling and were not prescribed calcium, contact your surgical team today. If you have tingling despite taking calcium; call your surgical team, your dose may need increasing.

Will my hoarse voice after thyroid surgery recover?

In the vast majority of cases, yes. Hoarseness after thyroid surgery is caused by temporary swelling or handling of the recurrent laryngeal nerve, which controls the vocal cord on the operated side. For most patients, hoarseness resolves within days to weeks. Persistent hoarseness beyond 4&#8211;6 weeks should be assessed by your surgeon, who may arrange ENT (ear, nose, and throat) review. Permanent voice change occurs in a small minority of cases.

Do I have to take levothyroxine for life?

After total thyroidectomy: yes. The thyroid produced the hormones that regulate your metabolism, heart rate, temperature, and energy. Without a thyroid, these hormones must be replaced. Levothyroxine is identical to the hormone your thyroid made. Most patients on a stable dose feel entirely normal. After hemi-thyroidectomy (half removed), the remaining half may continue producing sufficient hormone, blood tests at 6&#8211;8 weeks determine whether replacement is needed.

When should I take levothyroxine?

Levothyroxine is absorbed best on an empty stomach, at the same time each day, 30&#8211;60 minutes before eating or other medication. Coffee, calcium tablets, and many medications significantly reduce levothyroxine absorption if taken at the same time: allow at least 30&#8211;60 minutes. Consistency of timing matters more than the specific time chosen.

How do I care for the scar after thyroid surgery?

The thyroid scar sits in the lower neck and is visible. Scar management from Week 3&#8211;4 significantly affects the final appearance. Silicone gel or adhesive silicone strips applied daily flatten and fade the scar. Sunscreen SPF 50+ on the scar whenever it is exposed to sun for at least 12 months, UV exposure in the first year permanently darkens scars. Most thyroid scars are thin and well-hidden in a natural neck crease by 12&#8211;18 months.

Feeling cold all the time after thyroid surgery. Is this normal?

Yes: particularly in the first weeks after surgery and while levothyroxine dose is being established. The thyroid regulated your body temperature. While the levothyroxine dose is being optimised, some patients feel colder, more fatigued, and mentally slower than usual. This typically resolves once the correct dose is confirmed on blood tests at 6&#8211;8 weeks. If cold intolerance and fatigue persist beyond this, tell your GP, the levothyroxine dose may need adjusting.
