What is DVT and why does surgery increase the risk?

The good news: DVT after surgery is largely preventable with the right measures, and NICE NG89 provides evidence-based guidance used across the NHS to reduce this risk.

Risk factors that affect your DVT risk after surgery

DVT risk after surgery is not the same for everyone. Published NICE guidance identifies specific risk factors that, when present, significantly increase the chance of a clot forming. The combination of procedure type and personal risk factors together determines overall VTE risk.

  • Type of surgery, lower limb and pelvic surgery carry the highest risk. Hip replacement, knee replacement, and major abdominal cancer surgery have the highest DVT rates. Day procedures and minor surgery carry much lower risk. Source: PMID 34133395 (JBJS 2021)
  • Duration of surgery — procedures lasting more than 90 minutes increase risk. Surgery under general anaesthetic for more than 60 minutes on the pelvis or lower limb is a specific risk threshold in NICE guidance.
  • Immobility after surgery: being unable to walk or move freely for more than 3 days after surgery significantly increases DVT risk. Early mobilisation is one of the most effective preventive measures.
  • Previous DVT or PE: a personal history of blood clots substantially increases lifetime recurrence risk, especially around surgical stress.
  • Age — risk increases with age. People over 60 are at meaningfully higher risk. Those over 80 have significantly elevated risk.
  • Active cancer or cancer treatment, cancer is associated with a prothrombotic (clot-forming) state. People on chemotherapy have particularly elevated DVT risk.
  • Obesity (BMI >30): a known independent risk factor for DVT, particularly in combination with surgery and immobility.
  • Dehydration, thickens the blood and is a modifiable risk factor. Adequate hydration before, during, and after surgery is important.
  • Thrombophilia, inherited or acquired clotting disorders (such as Factor V Leiden or antiphospholipid syndrome) significantly increase risk.

How DVT is prevented after surgery

VTE (venous thromboembolism) prevention after surgery is a core component of post-operative care in most hospitals. NICE NG89 sets out the recommended approach.

  • Anticoagulant medication — blood-thinning injections (low molecular weight heparin, such as enoxaparin or dalteparin) are the primary pharmacological prevention. Your surgeon will prescribe these based on your risk level, typically for 28–35 days after hip or knee replacement surgery. Source: NICE NG89
  • Anti-embolism stockings (TED stockings): compression stockings are worn to maintain blood flow in the leg veins. They work best when combined with other prevention methods and should be correctly fitted.
  • Mechanical compression devices, intermittent pneumatic compression devices inflate around the calves while you are in bed, mimicking the pumping action of walking. Commonly used during surgery and in the first days after.
  • Early mobilisation, getting up and walking as soon as safely possible after surgery is one of the most effective DVT prevention strategies. Even moving your feet and ankles while in bed promotes venous return.
  • Hydration, adequate fluid intake prevents blood from thickening. Aim for 2 litres of water per day unless your surgeon has restricted fluids.

Symptoms to watch for: DVT warning signs

  • Swelling of one calf or lower leg
  • Warmth or redness in one leg
  • Visible surface veins becoming more prominent

  • Coughing up blood
  • Rapid heart rate with any of the above

DVT in the calf is serious. DVT that extends to the thigh or pelvis (proximal DVT) carries a higher risk of pulmonary embolism. Do not wait for symptoms to improve on their own; seek assessment promptly.

Understanding DVT Risk After Surgery

Deep vein thrombosis (DVT) (a blood clot forming in a deep vein, usually in the leg) is a recognised risk after surgery. During and after any operation, several factors combine to increase clotting tendency: reduced movement, the body's healing response to tissue injury, and sometimes dehydration. Most DVTs are small and resolve without complications, but a clot that breaks away and travels to the lungs (pulmonary embolism, or PE) can be life-threatening. Understanding your risk level helps you and your surgical team choose appropriate preventive measures.

Why Surgery Increases DVT Risk

Three interconnected factors: known as Virchow's triad: explain why clots form after surgery. First, blood flow slows in the deep veins when you lie still during an operation and spend time resting during recovery. Second, surgery causes direct damage to blood vessel walls, triggering the body's clotting mechanisms. Third, surgery creates a pro-inflammatory, hypercoagulable (more clot-prone) state as part of normal healing. These three factors together mean that all surgery carries some DVT risk: even when everything goes perfectly.

Procedure Type and Risk Level

Risk varies considerably depending on what type of surgery you have. Based on guidance from NICE (NG89, 2018, reviewed 2024) and the British Orthopaedic Association, lower-limb orthopaedic surgery — particularly total hip replacement, total knee replacement, and hip fracture surgery: carries the highest DVT risk. Abdominal and pelvic surgery (bowel resections, gynaecological procedures, prostatectomy) carries significant risk. General procedures involving general anaesthesia and lasting more than 90 minutes also increase risk substantially. Day-case and minor procedures carry lower risk, but are not zero-risk.

Individual Risk Factors

Your personal risk profile matters as much as the type of surgery. Factors that increase individual DVT risk include: age over 60, previous DVT or pulmonary embolism, active cancer or recent cancer treatment, hormone therapy (including the combined oral contraceptive pill and HRT), obesity (BMI over 30), pregnancy or recent childbirth, inherited clotting disorders (thrombophilia), varicose veins with phlebitis, significant medical comorbidities including heart failure and inflammatory bowel disease, and dehydration.

DVT Prevention: What Your Surgical Team May Use

Prevention (thromboprophylaxis) is tailored to overall risk. Measures include early mobilisation: getting up and walking as soon as safely possible after surgery, which is one of the most effective strategies. Compression stockings (anti-embolism stockings, or TEDs) keep blood moving in the leg veins during reduced mobility periods. Intermittent pneumatic compression devices inflate and deflate around the calves to simulate the pumping effect of walking. Pharmacological prophylaxis: typically low-molecular-weight heparin (LMWH) injections or direct oral anticoagulants; may be prescribed for days to weeks after higher-risk procedures.

Warning Signs: When to Act Urgently

Symptoms that may indicate DVT include swelling in one leg (usually the calf), pain or tenderness in the calf or thigh (sometimes described as a cramp that persists), warmth and redness over the affected area, and a feeling of heaviness or tightness in the leg. A DVT can occur without obvious symptoms. If a clot travels to the lungs, symptoms of pulmonary embolism include sudden breathlessness, chest pain (especially on breathing in), rapid heart rate, and (in severe cases) collapse. Any of these symptoms after surgery warrant urgent medical assessment — call 000 in Australia or 999 in the UK, or attend the nearest emergency department.
